It's this ability to swing between real indignation and comic exaggeration that explains 🤮's popularity well beyond its original medical use.

🤮 joined 🤢 in the same wave of emojis dedicated to unpleasant physical sensations, offering a more intense, more explicit version of disgust. Like its more moderate cousin, its use quickly extended to the figurative register, internet users having adopted it to express strong rejection toward content or behavior deemed unacceptable, independent of any actual nausea.

🤮 is used for severe physical discomfort ("that plane ride, I was 🤮"), to express total rejection toward behavior or an opinion ("what he dared to say, 🤮"), or to exaggeratedly mock a scene deemed over the top ("they're too cute together, 🤮"). It often accompanies reactions to content deemed scandalous on social media.

Its intensity makes it an emoji to strictly reserve for informal exchanges: in a professional setting, even a light one, it risks seeming excessive or out of place, unlike more measured expressions of disapproval.
